Initial Assessment - What to expect

WELCOME! If this is your first appointment at Ethos Collective Physiotherapy, or you’re treating a new injury, please fill out the Initial assessment and consent form. Click “Contact” at the top of the page and from the drop down menu select “Book an Appointment” and follow the instructions to BOOK NOW! You will be linked to our booking platform and will be asked to create a patient profile. This is required in order to schedule you in for an appointment.

Please be ready a few minutes before your scheduled appointment time. Your initial assessment is a one-on-one session and will last approximately 1 hour in length. The first portion of the appointment will consist of collecting your medical history. The second portion is an objective evaluation. It includes a physical full body assessment as well as an evaluation of the associated condition. The third portion includes a thorough explanation of the findings/diagnosis as well as a treatment. The fourth portion includes a personalized exercise program and recommendations targeting the deficiencies noted during the assessment.


Billing

Payment and receipt for services are completed and provided at the end of every appointment. We do not direct bill to your extended health benefit provider. If you have extended health benefits, please submit your own receipt for reimbursement. We also DO NOT accept claims from Motor Vehicle Accidents (MVA) or WCB. Patients are required to pay for services upfront with credit card, cash or e-transfer. It is clinic policy to have a valid credit card saved on file. Your personal and billing information is stored in a very secure, encrypted software program.

 

CHECKLIST BEFORE YOUR FIRST APPOINTMENT

Forms

I have filled out the Initial Assessment and Consent Form online before my appointment so that my physiotherapist can review it before my scheduled appointment.

Be prepared

If the physiotherapist is coming to my home, I have cleared out an area of my house (ideally quiet). I changed into my physiotherapy clothes in order to begin right on time!

Be organized

I have written down questions I would like to ask my physiotherapist and I have compiled important documentation (i.e physician notes, referral or imaging), which could be of benefit for the appointment.

I will be punctual for the scheduled appointment. If I need to cancel an appointment, I will need to provide at least 48 hours notice, otherwise I may be subject to a cancellation fee.
